If your child needs over-the-counter or prescription medication, parents must supply the medication in it's original bottle. Prescription labels must be on all prescription medications with clear instructions and a current date. Parents can print the "Medication Administration Consent" and turn it in to office with the medication.
If your child has asthma and needs an inhaler at school please complete the "Asthma Action Plan" and turn in to nurse's office. (This form must be completed by physician)
The "Food Allergy Action Plan" must also be completed by a physician. This is for children with severe allergic reactions to foods requiring the use of an epi pen.
**Students may carry and self administer asthma medication and epi pens with proper documentation from his/her physician.
